What does this mean for new and current clients? If you would like to be seen by a medical provider for medications for your mental health needs, you now have the option of staying in house at Identity.

How do I get signed up? If you are an existing client, please consult with your therapist to get onto our waitlist. Or you can outreach our case coordinator, Liz at our main office number 970-433-4635 or through the portal below.

Does this provider offer hormone therapy? At this time, our nurse practitioner does not provide hormone therapy. We can continue to refer to our local recommended community providers for hormone therapy needs.

What if I'm not a current client, but would like to learn more about medication options only? Please submit an inquiry through our website below, or contact our main number listed above/below to learn more.

Can I do therapy and medication services through Identity? Yes!

What insurances are accepted for medication coverage? Currently, our provider is set up to take Optum, United Healthcare, UMR, and Medicare, Rocky Mountain Health Plans (medicaid), Anthem Blue Cross Blue Shield, and Aetna.

Can I be seen in office for medication services? At this time our provider is virtual only. So long as you are a resident of Colorado, you can see her through our secure video system from your smart phone or computer device. No downloads of specific apps necessary!
